 UIE will send you the corresponding pro-forma invoice.

To print the order form choose the File / Print option in the File menu

Sales terms:
Payment by bank transfer in Euros only. 
Additional bank fees chargeable to the purchaser.
Ordinary mail delivery included.
Prices net of VAT.
Dispatching of books on receipt of the payment as indicated on the pro-forma invoice.
